The Wells Road hospital uses a multi-disciplinary team (MDT) approach to providing holistic mental and physical healthcare (PHC). Our PHC service provides care and treatment across all 5 wards and is integral to maintaining parity of esteem, ensuring physical health is given equal priority to mental health. 

We recognise the importance of improving quality of care in healthcare. Using a patient empowerment strategy through NHS Innovation Accelerator (a service that supports innovations across England’s NHS for greater patient and staff benefit), service users, especially in those suffering with complex, long-term conditions, have access to a service that aims to improve parity of esteem and provide and promote quality of care in healthcare. This fits with improvement strategies championed by Royal College of Nursing to recognise and acknowledge ‘bridging the gap’ between physical and mental health.

As part of its 5 Year Plan, the Trust has identified that “tackling health inequalities is key to the delivery of our strategy”. “It is our ambition to ensure that everybody who uses our services has a positive experience and feels involved in their care and the decisions to support their well-being and recovery”.

It is our aim to:

  • Ensure equitable access, patient experience and outcomes for all of our service users
  • Put service users at the heart of decisions about their care, delivering high quality, effective care and personalised experience
  • Promote a culture of continuous quality improvement that to positive changes within our service

Through health promotion, the PHC services aim to provide equal opportunities for all patients, supporting each individual to realise and achieve their full health potential. The PHC team recently restructured to improve quality and efficiency through working in collaboration with our patients and multi-disciplinary teams, supporting and developing best practice through education, training, and involvement. The triage system that is now in place is most effective for meeting patient needs, allocating them to the most appropriate clinician in the shortest waiting time.

The Wells Road provides both in house and outsourced contracting services to meet all service user needs. These services include GP access, Podiatry, Physiotherapy and long-term condition oversight from specialist Practitioners.

At all times we aim to provide a wide range of services for our patients ensuring that the approach to patient care and treatment is consistent, that a high standard of care is maintained, developed and is constantly improving, adhering to and working with NICE guidance, the Five Year Forward View, as well as health of the nation, latest standards and guidance:

  • Providing full health reviews on admission, a full MOT on admission and every six months
  • Working with an active general practitioner and surgery
  • Taking action to reduce substance use with our occupational therapy and psychology teams and support to quit smoking
  • Tackling levels of obesity and working with a whole team approach, providing a whole hospital healthy lifestyles strategy
  • Improving levels of physical activity across our patient population, offering a variety of activities to support people's interests and physical health abilities, with our physical activity team
  • Monitoring antipsychotic medication health, improving medication concordance and reducing side effects and ill health, working with our pharmacy colleagues
  • Providing extended services such as physiotherapy, podiatry, management and health promotion in diabetes and asthma as well as general self-care
