We are delighted that our Community Stroke team has been shortlisted in the national Smarter Working Live awards, in the Digital Transformation category alongside Isla (a visual technology solution). After recently also being shortlisted in the national HTN Now Awards in the Connecting Health category.
We were shortlisted for both awards for how we use a visual technology solution called ‘Isla’ to share patient data between the Trust and our local hospitals, to support the continuation of care in stroke rehabilitation. The use of Isla has supported the sharing of relevant information to a patient’s stroke rehabilitation journey. This has included videos and images shared from ward to community to support timely and effective discharge home.

Stroke rehabilitation is a complex patient pathway involving several multi-disciplinary teams, so it is integral to have a secure and reliable mechanism for capturing and sharing data between organisations. The ability to review videos provides rich patient context and gives patients the opportunity to contribute directly to their care record.
