Celebrating Community Services Week!
As we celebrate Community Services Week, we shine a spotlight on the integral role played by our community services in Nottinghamshire and beyond. At Nottinghamshire Healthcare, our Community Care Group provides essential health services for people across Nottinghamshire and extends support to neighbouring areas such as Derbyshire and Leicestershire. Additionally, we collaborate with key partners, including Nottinghamshire County Council, Nottingham University Hospitals (NUH), and Sherwood Forest Hospitals (SFH), to deliver a comprehensive range of services to meet diverse community health needs.
Our Community Care Group offers a full spectrum of services aimed at supporting the physical health and well-being of people across all life stages. Here’s a closer look at the services we provide:
Our adult services encompass both nursing and therapeutic care delivered directly to patients in their own homes. This allows us to provide tailored care for individuals with long-term conditions, complex needs, or those recovering from illness or injury. By bringing healthcare directly to people, we aim to improve their quality of life and promote independence.
We offer a wide range of services, including healthy families and paediatric provisions, ensuring that children and young people get the care they need to thrive from an early age. Whether it's supporting new mothers or addressing paediatric health challenges, our team is there every step of the way.
In partnership with local hospice providers, our end-of-life services offer compassionate, holistic support for patients and their families. We focus on comfort, dignity, and emotional care during the final stages of life, helping people to spend this time at home surrounded by loved ones when possible.
Formerly known as community hospitals, our bedded rehabilitation services support individuals transitioning from hospital care back to their homes. This vital service helps people regain strength, mobility, and independence following significant health events such as surgery or serious illness.
We provide comprehensive healthcare services within the prison system through our integrated offender health program. This unique service ensures that offenders receive the medical and therapeutic care they need, promoting health and rehabilitation in a secure environment.
One of the key strengths of our community services is that most of our care is provided directly in patients’ homes. This patient-centric approach not only increases comfort but also makes healthcare more accessible to those who may have mobility issues or require more personalized care. Our services are managed both locally and county-wide, ensuring that we meet the specific needs of different communities while maintaining specialist support across the region.
