RECONNECT

RECONNECT provides time-limited support to individuals leaving prisons within Nottingham / Nottinghamshire or returning to Nottingham City or Nottinghamshire from other prisons within England.

The individual to be referred must have a health need or vulnerability that requires additional support to reconnect to healthcare services in the community. We do not provide care and cannot replace any existing treatment provider.

RECONNECT also operates a cross-border pathway for individuals returning to other areas in England. Please speak to RECONNECT staff regarding this pathway before you make a referral.

The team includes:

  • Team Leader
  • RECONNECT Practitioners
  • Peer Support Workers
  • Community Support Workers

 

Who does RECONNECT work with?

To be eligible for Nottinghamshire RECONNECT, the individual must be in custody and returning to Nottingham or Nottinghamshire.

The individual must have a health need: physical health, mental health, Neurodevelopmental Disorder, substance misuse, or complex needs, and they must be motivated to engage with services.

The individual will have a presenting health need or vulnerability that creates barriers or impacts on a successful resettlement plan, such as difficulties maintaining their own health and wellbeing in the community, struggling to manage appointments and/or exhibiting chaotic behaviour.

 

What support can RECONNECT offer?

Once the referral is accepted, RECONNECT staff will conduct a thorough needs assessment with the individual. It is from this assessment that a decision is made whether to accept onto the caseload or not.

If the individual is accepted on to caseload, RECONNECT staff will support them in a variety of ways that can include:

  • Advocacy
  • Preparation for release from prison
  • Support with transitioning into the community
  • Linking care with internal and external agencies
  • Peer Support
  • Support to attend appointments (we do not provide transport)
  • Building resilience, confidence & self-esteem
  • Promoting independence
  • Onward referrals to other agencies

Who can refer to RECONNECT?

Referrals can be made for remand and sentenced prisoners. Referrals can come from the prisoner themselves, family / carers, Prison and Probation partners (i.e. offender management), substance misuse services, prison healthcare, Liaison and Diversion services. 

Please contact us to discuss the referral. Please also try to refer sooner, rather than waiting until nearer to the release date, to assist us to plan and create a robust support package.
