Working with our colleagues and patients to transform community mental health services
We are transforming our community mental health services to provide a better patient experience. We are implementing nationally recommended Patient Reported Outcomes Measures (PROM). PROMs measure a patient's health status or health-related quality of life at a single point in time, and are collected through short, self-completed questionnaires.
As part of this, we are ensuring we involve patients and colleagues. We recently held a co-production workshop with colleagues from Adult Mental Health, Mental Health Services for Older People (MHSOP), Inpatients and Involvement partners. The workshops are used to explore our current care plan and start designing a new one that incorporates patient reported outcome measures and is person centred, looking at the whole person and not just their mental health needs.
This is the first of a series of workshops to co-produce the care plan document, including a focus on the language we use, and communication needs of all our patients.
Our Involvement partner input was invaluable during the session, and we would like to thank them for their contribution to ensure we get it right and design a care plan that is fit for purpose and meets the needs of our local population.
