Independent investigation into the care and treatment provided to VC - an open letter from Ifti Majid, Chief Executive, Nottinghamshire Healthcare

The publication today of this independent homicide report is a tragic reminder of the horrific events of June 2023. At this difficult time our thoughts are with the families of those who died, Ian, Grace and Barnaby, and with those who were injured, Wayne, Sharon and Marcion. No-one should have to go through what they have suffered, and we offer them our heartfelt apologies for the opportunities we missed. As a Trust, we should have done more to listen to families, colleagues, and local partners while Valdo Calocane was using our services.

I want to be absolutely clear that we accept in full the findings and recommendations of the Theemis report. It is the third report concerning this tragic incident and it builds on our own serious incident report and the Section 48 review carried out by the Care Quality Commission.

Colleagues working across our services have been working to deliver an improvement plan, which is now yielding results and gathering pace. Today I am making a personal commitment to give a public update within the next four months on the progress we have made so far.  

We have achieved the national target of seeing 95% of all people who are referred into our services within 18 weeks. Every patient like Valdo Calocane, with severe and enduring mental illness, now has a risk assessment and a safety plan. Patients transferring between services can now expect a much more robust discharge process.  

Colleagues in the Trust deliver some incredible services that have a positive effect on the lives of thousands of people every day. I'm proud that they work so hard on improvement projects while treating and caring for patients with such kindness, compassion, and dignity. 

Today will be an incredibly difficult time for the families impacted by the Nottingham attacks. I can only imagine the grief that they will be feeling, and I am truly sorry for the missed opportunities that contributed to the tragic events of June 2023. 

We hold those events at the forefront of our minds as we strive to make changes and improvements throughout our services. The Trust is not in the same place that it was previously, but we are aware we still have much to do.
