Heart Failure Services

We provide care and support to patients with heart failure.

 

Accessing our service

All patients must:

• be registered with a Bassetlaw GP
• have a confirmed diagnosis of heart failure on echocardiogram

The service is available Monday to Friday, 8:30am to 5:00pm excluding bank holidays.

The service is based at Retford Primary Care Centre.

The Heart Failure Nursing Services provide a community focused specialist service to support people living with heart failure.

The aims of these specialist services are to work in partnership with primary and secondary care health professionals, using evidence based guidelines, to improve quality of life, reduce unplanned hospital admissions and facilitate end of life choices among heart failure patients.
